  • How much does it cost to get medical marijuana card in Arizona?

    There is a flat rate of $150 to receive your first identification card once you have been approved as a qualified patient. Each year renewal of this card is required for the same $150 fee. If the qualifying patient participates in the Supplemental Nutrition Assistance Program (SNAP) and has the proper documentation then the fee is reduced to $75. If a card is lost or stolen the cost of a replacement card is $10.

    What documents do I need to get my medical marijuana card?

    You will need to submit a copy of an one of the following:
    • • Arizona license or Arizona ID card issued on or after Oct 1, 1996
    • • Photograph page in applicants US passport

    • Arizona license or ID card issued before Oct 1, 1996 & one of the following:
    • • U.S. Certificate of Citizenship
    • • U.S. Certificate of Naturalization
    • • Birth certificate verifying U.S. citizenship
    • • A signed and dated Medical Marijuana Patient Attestation

    Photograph taken within 60 of the application submission date, in natural color, with no obstructions to the patients face or hair, with an off white or white background. The size of the photograph must be 2 inches by 2 inches.

    Note: SNAP documentation if applicant is applying for a reduced fee.

    Payment: A credit or debt card or pre paid cards are all acceptable forms of payment.
